How to create your form

Updated today

Head to Content > Forms in your Admin Portal and click "Create your first form" to get started.

  • Choose a form from the template library and customize it

    or

  • Create from scratch


Adding blocks

A block is a piece of content, used to build your form.

Forms uses a drag-and-drop builder with a full range of question types:

  • Multiple choice: one answer from a list of options

  • Checkboxes: one or more answers from a list

  • Dropdowns: a compact selection menu

  • Rating scales: numerical or star-based scores

  • Open text: free-form written responses

  • Matrix grids: rate multiple items on the same scale

  • And many more!

You also have layout and embedded blocks available, such as:

  • Paragraph, to write a short note

  • Divider

  • Thank you page

  • Image, video, audio, embed external content via iframe

  • etc...


Personalize your questions

Once you have added a block, you will be able to phrase your question and the each of the option's value.

Don't forget to save as you build! 🙂


Block settings

Blocks can be:

  • Deleted

  • Duplicated

  • Required

  • Hidden

  • Moved through the page to rearrange the order

To select a specific setting, click on the square, next to the +, to open the menu.


Conditional logic

You can add conditional logic to show or hide specific questions based on how someone answered a previous one — keeping the form relevant and focused for each respondent.

Access this feature next to the "Save" button, at the top right of the form.


Brand your form

  • Add a clear title and description to inform people why you are collecting this data.

  • Add a cover image to give your form a polished, on-brand look.


Preview your form

Once your form is finalized, you can see how it looks like with the preview option.

Access this feature next to the "Save" button, at the top right of the form.

You can reply to questions to test them out and your answers won't be taken into account.


Tips

> If you do multiple pages, members will be able to visualize the progression of their submission.

> If you want to come back to another version of your form, click on "Version history" next to the "Save" button.

You'll be able to select the version you need and restore it!
